...I would have assumed that the nipple cover was part of your concept, and a good one at that: a wee twist on the nude genre giving it a fresh look. Apparently, it's not uncommon for great inventions to be a result of an accident or a mistake ...or a model being a "prude" ;)

Most of them have a delicate HDR quality to them, giving them just a hint of painterly look but otherwise almost natural appearance, a refreshing change from in-your-face aggressive processing (which has its place as well, just happened to be widely abused), so if you don't mind sharing...

Is this how it really looks, or is the effect exaggerated (if not created) by a combination of the vantage point and lens's perspective distortion or in post processing?

...looks like you were just a tad to the right off the center line (neither obvious nor distracting though, so nothing to worry about :)
How did you deal with lighting? -- it looks very even throughout, including the high vault.

...don't know why but somehow I prefer this tight crop -- feels more "to the point" or "focused" I guess, for lack of a better term -- which is not to say that I don't like the uncropped version (as a matter of fact, I was debating which one to post for quite some time and, failing to make up my mind, I eventually decided to post both ;)
I'd be interested to know how you compare the two (?)

Multiple shots with a hand-held flash pointed in different directions
-- with the main burst aimed at the organs to give them prominence
whilst maintaining the overall darkness of the interior -- combined in
a stack (stack mode set to 'max' to leave the brightest pixels only;
and to 'min' for stained glass windows). Roughly a dozen exposures of
1/4s @ f/8, ISO 100.

Wow, I'm impressed how well you've composed without a tripod!
On a different subject, how come you've submitted this photo to the November Monthly Project: The Kiss -- interesting as it is, it has nothing to do with the theme, lol.
