It's creepy but interesting and I think well done.

I do think that the proportion of the skull is a little off.  The molars should be even with the pupil centers on both sides and the bottom of the eye socket would be about where the "bag" line would be if the subject had any.

.. and a  little scary. I think the skull looks very believable. I like the space to the left but not so keen on the top and bottom borders.

The crop seems good to me and the rectangular format works well as does all the black. I like the way the highlights of him arm seem to move toward his very bright (but not too bright) hand. That's quite expressive. I, too, am not that fond of the top and bottom white key lines. They become a big presence and I'm not sure they fit the bill. I love the fingers of his left hand just barely peeking out from under his right arm. Very delicate. The eyes are natural, well lit, and engaging. I could see this working with or without the skull.
