Charles, Your lighting and exposure are fine.  I just don't understand why you used a horizontal format and included the two glaring lights and cut off the top of her head.  For many years the horizontal format was called the "landscape" format and the vertical the "portrait" format.  I suspect it was movies and widescreen TV that messed this up.
