leaving some of the noise , and unsharpen wouldnt of hurt.

Compositionally this is lovely. The expression you have induced on the subject is worth a hundred words. I like the subjects posture too. Old faces with their wrinkles are almost timestamps and a face often tells more than a story. The ashtray in front also add another bit of the story. What would have made me enjoy this image quite a bit more would be if it was less saturated and your contrast was more like how it really was. The hyper-real is not adding anything to the image but taking away some of the charm.
