"Taking the Leap: Building a Career as a Visual Artist" by Cay Lang (a fine art photographer) ISBN 0811850935 Chapter 2 "Creating Your Artist's Packet" is 37 pages on preparing a portfolio for querying galleries and adapting it to the needs and instructions of the galleries you choose to approach.
