This morning, in a Billingham Hadley Pro, my D700 with 24/1.4. Was at the mall, and came home without any shots
In the evening, had a family dinner, so went with D700, 24-70/2.8 and SB-800 in the Hadley Pro. Lots of family pics taken over dinner and the subsequent birthday cake cutting ritual

To tell the truth I don't carry a bag. I have a D2h with Sigma 120-400 on it in a Tamrac holster, A D2h with Nikkor 80-200 f2.8 on it no case, a D2x with a Sigma 18-50 2.8 Macro in a Tamrac holster. The Tamrac Holsters have a spare battery and spare memory in the pockets on them. I have a Victorinx case with all the associated manuals,charger,ac-adapter,Giotto Q-ball,strobe and cleaning stuff that is usually carried in the trunk of car with tripod and monopod. Last trip by air I carried the D2x on and charger and stuff went in my checked bag. No spare lenses except I did put a Sigma 105 Macro in my checked luggage. It's becoming almost impossible to fly with any real amount of gear and that's sad.

A&A Evans Walker with M6 plus 50mm 1.5 C Sonnar loaded with Superia 200; M2 plus 35mm f2 and cap. Hoods for both of those. No hood in the bag unfortunately for the Zeiss 25mm Biogon, but the Zeiss finder is there and front and rear caps. So is the SHOOC 135mm finder, superfluous. Blower brush. Gossen Digipro F light meter. Cable release. Rollei Retro 100 and another roll of Superia 200.
The other bag has the OM2n with the 50mm 3.5 macro, the f4 200, a 50mm 1.4 and a 28mm 2.8. And a Leicina Super 8 movie camera. Caps, hoods, photocopied instructions for the movie camera.

I'm a Canon guy, but I want to join in the fun too! In my little Lowepro waist-pack I have a 5D, a 24mm f/2.8, a 50mm f/1.8 II, a 100mm f/2.8 USM Macro, and if I've packed it right, a 420EX flash too. In the extra pockets I have a set of Lee ND grads, a Lee system holder, a set of Lee ND grads, and a B +W circular polarizer. Slung over my shoulder is a Manfrotto 190CXPRO3 tripod with a 488RC4 ball head on top.
Truth be told, often the Lee stuff and the tripod are left behind, but when I'm feeling energetic, that's what I bring. I don't have a car at the moment, so I walk with it all, use my bike or bus it.

Pentax K-7 with DA* 16-50mm F2.8 mounted.
DA 12-24mm F4, DA 55mm F1.4, DA 35mm F2.8, Macro Limited and DA* 60-250mm F4.
Depending on the shoot, I more often take the DA* 50-135mm F2.8 (more people) instead of the 60-250mm (more wildlife). Carry both would mean more visits to the chiropractor ;)
Sometimes use the FA 31mm F1.8 instead of the 55mm for something wider if do not expect to need the extra stop and it is not raining.
77mm filters: Hoya Polarizer HD, Hoya ND8, Hoya ND400, UV (for imminent danger only: salt-water, flying sand, etc). Step up rings to 77mm for all zooms with different thread size.
Remote-trigger, 3 batteries, Gary Fong Puffer, 4 x 2GB memory cards, memory wallet. Lens pens, two micro-fiber cloths (one for the lens one for the viewfinder/LCD), lens tissue, lens cleaning fluid. Extra quick-release plate.
...Now that I wrote all this, I did not realize my Lowepro Nova 5 could carry that much!
