amorteguy
Glazer's in Seattle will rent and ship after the paperwork is complete (at least they told me they would, and I live in San Diego. I ended up buying and not renting, so I never took them up on this.)
michaelbrochstein
I live in New York City and recently rented a Nikon 200-400 for a trip to Alaska (coincidentally!). They shipped the lens to me, I went on the trip and then shipped it back to them. We each used overnight (FedEx?) shipping and everything went very smoothly.
