I have an old style 645 that does the same thing. Your right about pushing the insert in firmly but at times that doesn't work either.
I sometimes have to turn the counter gear inside the camera and roll it back to the correct start position. It does not happen all the time
on my camera so it may just need to be cleaned. Let me know if you find a fix for the problem. I hope this info helps.

I HAVE A DARKROOM AND PRINT SOME EVERY WEEK, BOTH 21/4 X 21/4 AND 35MM AS A HOBBY FOR ME AND MY DAUGHTER. I USE A MINOLTA TWIN LENS AUTOCORD AND WOULD LIKE TO MOVE INTO A "SLR" MEDIUM FORMAT WITH INTERCHANGABLE FILM MAGS. I'M BY NO MEANS AN EXPERT IN THE MEDIUM FORMAT SLR's AND WOULD LIKE SUGGESTIONS ON WHAT USED EQUIPMENT TO LOOK AT. FOR EXAMPLE I SAW A MAMIYA (OLD SYLE) 645 W/80MM F4.5 LENS, FORSALE AT $495.00. IT SOUNDED GREAT BUT WHAT IS (OLD STYLE), MANUAL?? AND DOES THE OLD STYLE HAVE INTERCHANGABLE FILM MAGs??? AM I LOOKING THE WRONG DIRECTION???
IS BRONICA, PENTAX, WHERE IS A GOOD PLACE TO START?? WHERE CAN I GET SPECS ON THE OLDER SLR MEDIUM FORMATS TO SEE WHAT WAS ONCE AVAILABLE??
Minolta Autocord TLR service manual

Does anyone have a service manual for a Minolta Autocord that I might get copies of??????????? I have emailed Minolta about the shutter needing repair and they recommended it be replaced. I do not want to discard the camera because the shutter problem is only problem I have had with it in 15 years, it has a few more shots left in it.
