1.
The problem with the lens ID's is that Sigma re-uses the same ID for many different lenses so the focal length and aperture must be used to distinguish between the possibilities. The latest version of exiftool includes the 100-300 so it should get it correct (for most focal lengths).

 2.
It looks to me like the D90 always stores "00" for the sub-second values. This isn't true for all Nikon DSLR's though. Here is a random sampling from samples I have handy (table of SubSecTimeOriginal and Model information):
3.
I'll try to clear things up:
In general, if you want to write a lens name to an image, you should use the XMP-aux:Lens tag.
The only writable "LensID" tag is XMP-aux:LensID, which accepts only integer values. The Composite LensID tag is not writable -- to set this you must write the LensType tag from which this value is derived.
You can write Pentax:LensType, but only to a Pentax image, and only with a lens name which is in the lookup list (see the Pentax tag name documentation). Since the lens you want to write is not in the lookup table, then it can't be used to set the Pentax:LensType value. The closest lens in the list is "smc PENTAX-FA 35mm F2 AL".
I hope this helps.
