This shot is just amazing. I too love the focus on the hands. I agree that that spot of light on the man's ear is a little distracting, but I disagree that it looks violent. Her hand is relaxed as is his. If it was "forced" looking it would seem her nails would be turned inward toward his flesh.
Her head being turned doesn't signify unwillingness to me either, but quite the opposite as he is not forcing her head to the side. She is willingly turning her head to allow him to kiss her there.
I do agree though that the woman's facial expression leaves something to be desired.
