I was in Bhutan a few months ago... All my travels were made thru a Bhutan agent (which I belive is a requirement... even if you book with a company in the US, that agent will have to work with a Bhutan travel agent I believe)... In terms of the minimum per day, one should consider that this normally includes food and lodging too, for me it was not a flat fee charged by some gov't agency. My trip was not a trek, but in the banter during the trip the same principle seemed to apply to tourists trekers too in that the gov't wants to make sure that there is enough rooms for all tourists, not limit the number of people coming into Bhutan. Our guides and Bhutan TA's told us that there is no 'quota', only a system to insure there are enough resources to handle the tourist traffic.
