Doubt evolution

I have difficulty to understand this image, even if it has made itself to observe for along time. I find and introvert dramatic. Which it is the doubt and which it is evolution? I see a face deformed from a dense and full liquid of energy. Perhaps but it is only my fantasy or desire to understand.
