try PS clone and remove the white area just to the upper right hand side, it will improve the portrait without taking anything anyway from the impact of the image

Don - thanks very much for the tip. It's often all too easy to overlook obvious corrections as I have in this instance.

The effect is a combination of low light shooting and then working with three layers on shadow/highlight, diffuse glow and curves - along with the usual adjustment of levels and a final sharpening.

 

I just play around with the blending options and opacities until I have a result I am content with.

I find it looks exactly how it was described; very overprocessed, giving the party the look as if he's suffering from a fatal disease that will kill him in the coming hour.

Given that - clearly - the aim was anything but naturality, your comment indicates a successful outcome.
