This was taken nearly a year ago, but if I remember correctly, it was a 3 light set-up. Starting on the left (the model's right) was a light set at around 45 degrees to her face set at her head level. In the center (i.e. right below my camera) was a fill flash with softbox for shadow removal. Finally, there was a hanging light with barn doors on the right (model's left), 45 degrees to her face.

You would have thought that the colours would have clashed here. Perhaps they do but you get away with it.

 

My only niggle(s) are the joins at the back. (You'll probably PS them out now and resubmit the photo.)
