What a strange image, Michael. It doesn't look like an obvious photo manipulation but then how to explain the hand reaching out to the woman's face. At the very least an intriguing puzzle of a picture. Enquiring minds want to know!

There is some sort of unexplainable tension here...

It looks to me like a reflection mixed with original image, and everything done with just one shot.

That's the beauty of photography!
