Michael, I much like the "base material" of this image; the composition, the choice for B&W, tonality - all good. As a scene, it works, it's got nice interaction between the two, and it leaves a lot of room as a viewer to project into this image. And I think you can feel the "but" coming. There are two things that distract me.

First, and this is minor, is the highlight on the forehead of the "patient"; it looks a bit unnaturally white. Second, and to me more important, the background. Not sure if you made the "frosted glass" look, or whether it was there originally, but it is to me highly distracting. It doesn't help focussing on the real subject, rather it draws away from it. I think a normal blur (less textured) would work a lot better.

As always, I am grateful for your thoughtful comments.  I'm not sure how to account for the light area on the "client's" forehead, except to say that the lighting in this area was very spotty and inconsistent.  As for the background, I chose to present it this way - somewhat abstract - to suit the "therapeutic" setting.  But I do get your point.

Michael,

A very interesting and very different photo. It stimulates much thinking in the viewer's mind. I think the background just adds to the mystique of the photo which works well in B&W. I suspect it would not hold together if presented in full color. Nice work here Michael!
