Wonderful utilising of window lighting and very effective DOF in order to obtain all of this remarkable details of the place contents, I also like the tone which do add a sense of drama to this amazing image and enhancing its composition.

Alon, the heightened contrast between darkness (and disarray, filth, dilapidation, and rust) compared to very bright sunlight seen from a window that shows no outside --yikes -- makes for a clinic like a dungeon --  I suppose Teresin? 

Maybe too much contrast, maybe not! Worth trying to decrease it and see how would it look then!

Heartstopping use of the light.  It recreates a sense of desperation quite adeptly.
