background is too far out-of-focus and you can' t tell... :)

She has a very nice smile. The lighting and her skin tone looks good. You have focused quite well on her eyes.

The background is nicely out of focus, but it is still quite cluttered with colors, shapes and "things" that draw my attention.

Her chest is lighter than her face. You don't want anything to draw attention away from her face. I think her face could be a tad lighter.

I would give her a little more headroom. She seems too close to the top.

She could use catch lights to give her eyes life and I would soften the dark circles under her eyes.

Don't pose her straight on to the camera. If you turn her at an angle to the camera you wouldn't be showing her widest area -- shoulder to shoulder. She would look thinner and more feminine at an angle to the camera. The lines you see from an angle have more apparent motion, interest and grace. Somewhere around 45 degrees to the camera usually works quite well.

I agree with you that her dress (waitress uniform?) is not too stylish looking.

I agree that the posing is a little too flat to be flattering. But maybe that's not the effect you were seeking.
