Let me add to the accolades above of it being very dramatic exploration and depiction of 3-dimensional shape.

I like the exposure where it is. Think the white of the cigarette works here.

Prominance the hand is good graphic hook. Taken close with a normal or shorter lens?

Natural that no smoke is visible -- it is being inhaled as evidenced by the concave cheeks.

Like the white hair on LHS - makes the pic work with black background when many don't.
