It has to go, I can't believe anyone would take a shot with a fly on the kids face. The hair is a little messy but as it's a boy you could expect that, other than that it's not a bad shot.

As far as cropping and PS is concerned the sky is the limit when you have a photo. Here's a mild alteration

I LOVE the fly! I love the picture! Rename it from a "portrait" (as in a stodgy old geezer, puffing himself up in a stiff pose) to a "candid" and now you have captured the essence of a three-year old boy: "Frogs and Snails and Puppy Dog tails..."

 

The lighting is great, too! Did you use fill flash? Whatever you did, keep doing it!

I agree with Keith. The fly, while a rare snap doesnt seem to have the dynamic grab value to the observer as the magnificent eyes. To me that is the money target in the shot. And consequently they should be brought to primary focus. I would dump the fly. Although the hair on the head hold some interest if you crop to include the whole head ( and I think cropping will do alot to redefine the eyes) that places the eyes below center in the frame. (as in left pic). Therefore I recommend a closer crop drawing focus to the eyes, those great eyes. (as in right pic)

Poo poo cropping this...and don't you dare run this through photoshop! My only wish for this shot would be a bit more background. I mean if there were more canvas to the right of the face it would add so much to the composition--that you could do in photoshop.

 

But really, leave the fly otherwise it's just another picture of a rugrat.
