I suspect some "smoky" filters were applied to layers... or maybe even gradients? The blur was added, as you said. In short, that is no photograph. It is photo-based art. It looks like to me either the person in the portrait is very short, or McCurry was standing above her, which might contribute some to the funny DOF. Also would explain the catchlights in her eyes, since she would be looking up.
