Two possibilities:

 

1) GoCubic - see http://www.panoguide.com/howto/display/gocubic.jsp

 

2) Use the Adjust plugin from Panorama Tools to convert from cylindrical to a 360 x 180 equirectangular format. Then use PanoCube to convert the equirectangular image to a cubic qtvr .mov file.
