proof cd, a statement to include

I'm looking to include a written statement with my proof cd I give to clients.

I am giving them "print only, for personal use" permission. But I wanted it to

say a few other things like....I cannot guarantee print or color quality with

other labs but my own, they are responsible for preserving their own images if

say, cds or dvds become obsolete, and anything else you can think of...

Anyone have an example with fancy wording?

We got examples with the legal forms book for photographers and various samples on internet forums.

You might want to include those statements in the actual contract so that they are agreeing and understand those terms before signing with you, that's what we do.
