Old B&W photo - help with restoring complicated part

The section below shows the neck which, as you can see if pretty

badly "broken". I'm trying to retouch in PhotoShop but can't seem to

get the shading right - I have used the clone rubber stamp tool so

far. Any help with the workflow to achieve the right result would be

vastly helpful.

 

Also, is it possible to get rid of the dark patterns all over the

face and replace with a smooth texture, without losing any of the

shading detail.

There was a good thread on softening skin in Photoshop, I think in People Photography. In there, there was a link to a Carol Steele technique, which I thought works nicely. Photoshop's healing brush and patch tool have proven themselves as in the previous examples. As for the dark patterns and spots in the skin, which I treated as noise, I suggest the best tool for that job is a median filter. Photoshops median can handle selections of images well, but will quickly ruin an image with lots of defined edges. Noise reduction software will do a decent job of smoothing out the tones, but usually softens edges undesireably. Reindeer Graphics produced a PS plugin called Optipix, one of whose many useful filters is called 'safe median'. This is what I ran on the image. I think you'll agree it does a fantastic job of retaining edge details while minimizing noise characteristics. I'm impressed with Optipix. Daniel's results were better than I could pull off with Dust and Scratches.

 

Neat Image, on the other hand, surprised me. The attached is that, with the usual tricks. BTW, I think the thing in the left corner is a necklace, not a scratch.
