Model release form - Have one I could use?

Hey People. I'm soon going to be entering into a shoot involving

alot of relative uknowns, ie, kids doing extreem sports well, but

not professionally. The work however may be used in a marketing

aspect. I do not have any model release forms of any kind. I, in

fact have never even laid eye's on one before. It may be a shot in

the dark but would any of you have a model release form that I could

use? I know it's a big question to ask, but I just had to. It would

be greatly appreciated. Thanks.

Just google the phrase "model release form" and you should find some examples that you can adapt.

Here's a very basic one.

MODEL RELEASE

Date:_________ Place _____________________________________________

I, ___________(models name)__________________________________________

(Please Print Clearly)

hereby grant, ________(photographer's name)__________________________

permission to use, copywright and publish the photographs taken of me today for purposes of (insert your purpose ie editorial, advertising trade etc) in all lawful media.

Signed __________________(models signature)________________________

...and, if you are dealing with 'minor' kids, in most of the U.S., you need a Mom or Dad (or legal guardian) to sign the release. The kid's signature is nice, but not worth much if Mom, Dad, or the guardian determine they would like 'payment' for the use of their kid in any form of advertising or marketing. A newspaper article does (generally) not require a release.
