1. There's nothing worse than coming to a similar situation, finding a thread aaaaaand zero follow up.
 2.
The camera was returned from David Odess and it's now in perfect focus. Aside from the full CLA on the body and a repair to one of my A12 backs, an adjustment was made to the focus screen height. His knowledge is vast on these camera bodies and backs. I'm glad to have the camera back and it's smoother than ever!

 3.
The 501 also uses a different mirror setup. IMO, the 500c/m is as good as long as it has maintenance.

 4.
While I can't speak regarding the 150mm, I use the 42170 acute-matte focus screen and, combined with the magnifier, can get razor sharp (when my camera is returned from David, this is how I discovered the focus issue) using the 80mm T* at f/2.8.

 5.
From what I've been told, the mirror pads keep the mirror from any movement. I probably have the mirror pad issue and I'm hoping there are better materials available now that might do a better job and resist deterioration.

 6.
If you focus on an object at infinity, is the split image correct or still split? This is how I discovered my focus issue before sending it off to David.

 7.
I just sent off my 500cm to David Odess for the same, exact issue.

 8.
I'm going through the same problem at the moment. I have sent off my body and A12 back to David for recalibration. I'm almost positive it is the mirror assembly, the pads under the mirror, or the resting ramp. He will figure it out.

 9.
I managed to grab a few shots of the back focusing issue. I'm sending the body into David this week as soon as the holiday craziness subsides.

https://drive.google.com/file/d/0B3NoS-dIsGMBWHNhSlFUZGhMMEE/view?usp=sharing

 10.
Hello,

I'm new to the site but have been searching for a few days to see if anyone had posed this question.

The issue was the mirror pre-release mechanism not working. I have since fixed the issue by disassembly and using Dick Werner's instructions, pried the lock up lever while re-assembling the body. I took the opportunity to lube the main gear and a few other spots he recommended with clock oil. The body is now extremely smooth and snappy.

Since I acquired the body, lens and back, I've noticed the focus a bit off. I'm aware of the 3 pads under the mirror issue and I am going to be sending the camera in for a CLA, but I wanted to see if the issue could also be caused by the malfunction of the mirror pre-release lever.

I will be taking some shots using a back/front focus test chart I use for autofocus lenses, and just need to develop them so I can use the images when the camera goes in for service. I figure it will help the tech narrow down any issues.

Thanks for any information and a great resource (site).
